Program Report

The recent years have seen a growing number of community-based initiatives that are integrated with pharmacy services to promote health management, safe and effective use of medication, and to empower patients in prevention and management of chronic diseases.


With the evolving models of medical-social collaboration and public-private partnership, there are vast opportunities to strengthen primary care provision with pharmacy services. However, community-based care providers face multiple challenges in service planning and implementation, such as resources barrier, the lack of established model, scalability and sustainability.

The Primary Care Pharmacy Consortium 2019 aims to bring community providers to share their experiences, and to discuss the opportunities and challenges of local primary care pharmacy services. It also aims to facilitate networking and to foster further collaborations on:

  • Planning, implementation and evaluation of primary care pharmacy services;

  • Research to guide evidence-based pharmacy practice in primary care setting;

  • Education to nurture the next generation of pharmacy leaders in primary care.
