What is Medication Management Services (MMS)? 

MMS is a person-centered community pharmacist-led service in optimizing patient’s medication use and improving health outcome by focusing and reviewing on the accessibility, appropriateness, effectiveness, safety and adherence of the medication therapy. 

The service aims – 

  • To empower patients and/or their caregivers with better knowledge and literacy in self-management, health-related decision-making and health promotion 
  • To optimize and review the accessibility, appropriateness, effectiveness, safety and adherence of medication therapy of the patients 
  • To support patients and/or their caregivers in a holistic healthcare approach (e.g. addressing psychosocial concerns of individuals) through better utilization of other community resources 

What issues could Medication Management Services address? 

Community pharmacists help to handle enquiries and resolve difficulties in medication management, for example: 

  • Effectiveness and side effects of medications 
  • How to manage adverse drug reaction 
  • Tips on proper use of medications 
  • Consolidate medication history 
  • Potential drug interaction 
  • How to manage missing of dose 
  • How to schedule time to take the medications 
  • Whether medications be cut or crushed 
  • How to store the medications 
  • How to handle expired medications 
  • How to make good use of pill box and technology for health management 

How do community pharmacists provide Medication Management Services? 

MMS consists of the following service components: 

  • Collect clinical and medication history from patient and/or caregiver and eHealth 
  • Compile the best possible medication history 
  • Review the medication therapy, including the accessibility, appropriateness, effectiveness, safety and adherence 
  • Identify any medication therapy problem(s) and recommend professional clinical interventions 
  • Make shared decisions with patient and/or caregiver on the plan for resolving medication therapy problem(s) 
  • Provide education, counselling and necessary support to enhance medication use and disease management 
  • Formulate care plan with personalized recommendations on medication use and lifestyle modification in order to optimize disease management 

Expected Outcomes 

  • Individuals would be aware of the benefits of good health and medication management, and their potential capability to manage their own health, and thus encouraging them to proactively join the decision-making for their health planning. 
  • Individuals would acquire essential knowledge and skills to handle health- and medication-related issues and have a better understanding of handling future health-related needs. 

Service Target 

This service targets individuals who are: 

  • Polypharmacy (Taking five or more chronic medications) 
  • Taking high risk medication(s) such as antiplatelets, anticoagulants, antidiabetic medications, drugs used in central nervous system and immunosuppressants 
  • Having changes in medication therapy recently (e.g. recent hospitalization, initiated on new medication)

社區藥劑師如何提供藥物管理服務? 

藥劑師會 – 

  • 搜集服藥人士的臨床病歷、藥物記錄 (例如向服藥人士及/或其照顧者查詢、查閱醫健通) 
  • 整理最新的藥物清單 
  • 檢視藥物治療,包括可及性、適當性、有效性、安全及藥物依從性 
  • 針對藥物治療問題,提供專業臨床建議 
  • 與服藥人士及/或其照顧者就解決藥物治療問題的計劃,作出共同決定 
  • 向服藥人士及/或其照顧者作出個人化藥物及生活模式建議,共同制定健康管理計劃 

預期成果 

  • 參加者能了解到良好的健康及藥物管理的好處,並能發掘自我健康管理能力,從而更積極參與有關個人健康的規劃。 
  • 參加者能學到處理健康及藥物相關問題的基本知識和技巧,並明白如何應對未來健康的需要。 

服務對象 

本服務適合以下人士: 

  • 多重用藥 (正服用五種或以上的長期病患藥物) 
  • 正服用高風險藥物,例如抗血小板藥、薄血藥、降血糖藥、精神科藥物和免疫力抑制劑 
  • 藥物療程於最近有更改 (例如最近住院期間有更改、獲處方新藥物)
