Fostering Transdisciplinary collaboration in Primary Care Pharmacy Service

Primary Care Pharmacy Consortium:

The Primary Care Pharmacy Consortium serves as a platform to foster transdisciplinary collaboration between the academia, social service and healthcare providers on:

  • Planning, implementation and evaluation of primary care pharmacy services;
  • Research to guide evidence-based pharmacy practice in primary care setting;
  • Education to nurture the next generation of pharmacy leaders in primary care.

Online Conference :

With the commencement of District Health Centres/Express, the recent years have seen a growing effort and interest in integrating pharmacy services into primary care service models to promote health management, safe and effective use of medication, and to empower patients in prevention and management of chronic diseases.
The conference aims to bring primary health service providers to share their experiences and practical aspects of pharmacy services, to encourage knowledge exchange among stakeholders, and ultimately to contribute to the sustainable development of primary pharmacy services through medical social collaboration.
