We are currently working on the resource sharing area in due course.

The resource sharing area will share practice-based resources to facilitate the delivery of various services related to medication safety in local primary care setting. Examples of the resources are as below:

• Charts, standardised medication reconciliation and review forms for healthcare professionals
• Patient leaflets for the service providers and the public

The practice-based resources are covered by the Creative Commons License, to facilitate their adaption to your services.
