This two-hour online session aims to enhance participants’ understanding and practical skills in medication reconciliation and review, focusing on community settings for patients with multiple chronic conditions and polypharmacy. It highlights the importance of medication reconciliation and review in preventing medication errors and improving patient safety, while discussing the specific challenges faced in community environments. Participants will explore tools and technologies to streamline the medication reconciliation process.

Furthermore, participants will gain actionable strategies and expert knowledge to enhance their professional practice. Featuring the insights and real-world experiences from a DHC pharmacist’s and a clinical pharmacist specialising in geriatric care, the seminar emphases collaboration among healthcare providers to improve patient outcomes, medication adherence, and reduce adverse drug events.
